When signing up for a family membership, start by clicking on individual/first member. To add an additional family member to your membership, you can either click on contracts -> whichever Barn Add On you'd like OR come back to this page and click each add on and it will be added to your cart on MindBody.

Joining is simple! Just fill out the forms below and return them to us before or at your visit!
On the membership agreement where it says "Services included in monthly package" simply write in the membership option you would like (i.e. $49.99/mo Individual). Then under monthly payment amount put the amount (i.e. $49.99). The second page has a place for credit card payments.
